Aggravation by Routine Physical Activity and Associated Factors in the Korean Migraine Population

Abstract
Background: Aggravation by routine physical activity (ARPA) is included as an item of headache characteristics in the migraine criteria of ICHD-II and ICHD-III beta. However, pathophysiology of ARPA during migraine attack is not fully understood, and the association between ARPA and other clinical characteristics of migraine has not been reported. In this study, we sought to evaluate an association of clinical characteristics with ARPA among the Korean migraine population.

Methods: We used the data from the Korean Headache Survey designed to investigate for primary headache disorders using ICHD-II criteria in Korean population aged 19-69 years. Data on age, gender, geographical region, education level, BMI, and clinical characteristics were collected using a 29-item, semi-structured interview. Migraine population was dichotomically divided according to the presence of ARPA. Logistic regression analysis was used to determine factors independently associated with ARPA.

Results: Among 1507 study subjects, 91 (6.0%) met the migraine criteria of ICHD-2 and 62 (68.1%) had ARPA. In univariate analyses, headache frequency, intensity, photophobia, phonophobia, and osmophobia were related to ARPA, and living small city and unilateral pain were inversely associated with ARPA. Multivariate analysis revealed that ARPA was independently associated with photophobia and phonophobia (OR: 1.34, 95% CI: 1.02-1.75; OR: 2.92, 95% CI: 1.13-7.54, respectively).

Conclusion: In this study, photophobia and phonophobia were closely associated with ARPA in individuals with migraine. These findings suggest that ARPA might be a sensory hypersensitivity symptom to motion or movement during migraine attack.
